- Take your large stack of medical records and compress it down to only the useful pages (this is usually less than 5% of the original document)
- Return copies of all the important useful pages with hand written notes
- Create a time line of the records/hospital stay
- Write a narrative summary in common easy to understand terms with page references
- Explain complicated procedures, course of care, medications, etc.
- Interpret what is written "between the lines" in the doctors or nurses notes
- Find missing or incomplete information that may still need to be addressed.
- Suggest recommendations based on clients needs (for instance in child welfare cases, come up with suggestions that should be met to support reunification)
- Suggest long-term issue/needs that could arise
- Offer a verbal interpretation and answer questions if desired
